Antibiotic Prophylaxis and Preventing Postpartum Infection

بحث بر روی کاربرد پروفیلاکسی آنتی بیوتیک ها در مامائی، در اواخر سال ۱۹۴۰ شروع شد و تجویز پروفیلاکسی پنی سیلین به تمام بیماران در لیبر باعث کاهش شیوع تب از ۶/۳ به ۴/۲٪ می شود. ولی هرگز آنتی بیوتیک پروفیلاکسی بطور عمومی مورد قبول واقع نشد. بدلیل اینکه اولا شیوع عمومی عفونت، پائین است و عفونتها بندرت شدید می شوند، بطوریکه بیش از ۹۵ تا ۹۷ بیماران احتیاج به آنتی بیوتیک پیدا نمی کنند. شاید، تجویز غیر ضروری آنتی بیوتیک ۴ - ۲ درصد سبب کاهش عفونت گردد، لیکن هنوز ۲ درصد مادران مبتلا به عفونت خواهند شد. دوم اینکه مصرف گسترده آنتی بیوتیک، خطرات مشخصی مثل عکس العمل داروهای کشنده و ظهور ارگانیزم های مقاوم را در بردارد.
